episode 3: How to understand VA Medical Records Structure

Welcome to The Veterans Disability Nexus, where we provide unique insights and expertise on medical evidence related to VA-rated disabilities.
Leah Bucholz, a US Army Veteran, Physician Assistant, & former Compensation & Pension Examiner shares her knowledge related to Independent Medical Opinions often referred to as “Nexus Letters” in support of your pursuit of VA Disability every Wednesday at 7AM.

In this episode, Leah Bucholz provides a comprehensive guide for understanding medical records, emphasizing their relevance in VA disability claims. She explains the standard structure of medical records, regardless of their origin. The records typically start with the chief complaint, followed by a section detailing vital signs, allergies, medications, and surgical history. The subjective portion covers the patient's specific complaints for that visit, including details like onset, provocations, severity, and timing. The objective findings section includes the doctor's observations, such as swelling, bruising, or other physical signs. Leah highlights the importance of the assessment or diagnosis section, where the doctor's diagnosis, crucial for VA disability claims, is documented with specific ICD-10 codes. Finally, she describes the plan section, outlining the prescribed treatment or interventions.
