Comprehensive biomarker profiling of complex and heterogeneous tumors such as non–small cell lung cancer (NSCLC) is crucial for determining the best therapeutic approach for each patient. In metastatic NSCLC, the number of molecular alterations that have FDA-approved matched therapies has been rapidly growing. The first targeted therapy has received regulatory approval in the adjuvant setting of early-stage disease, and there are many more emerging targets against which novel therapies have shown great promise in clinical trials. Patients with lung cancer who have targetable genomic alterations can derive remarkable benefit from these therapies, and thus, broad biomarker testing is essential to identify these patients, as is making the best rational therapies available to them. Many barriers to biomarker testing and appropriate biomarker-driven treatment exist, and there are also significant disparities in lung cancer clinical care and research, but effective steps can be taken to overcome them. An essential component of modern lung cancer management is patient education, which allows patients to become well-informed participants in their care and participate in shared decision-making with their multidisciplinary clinical teams.
