For clinicians managing patients with advanced ovarian cancer, the use of poly (ADP-ribose) polymerase (PARP) inhibitors and new treatment modalities such as tumor treating fields (TTFields) represents an important opportunity to provide personalized medicine. With a key role in the treatment and maintenance of recurrent disease, as well as in first-line maintenance of newly diagnosed disease, PARP inhibitors are indicated by the FDA for use in patients with BRCA-deficient tumors, homologous recombination deficient (HRD) tumors, and biomarker wild-type tumors. PARP inhibitors are also being actively researched in combination with other systemic agents, including targeted therapies, chemotherapy, and immunotherapy, in an effort to overcome PARP resistance and exploit additive efficacy. Already approved in glioblastoma and mesothelioma, TTFields, a new modality that targets solid tumors via alternating electric fields, is paving the way for improved outcomes in a variety of cancers, including ovarian.
